I would have thought that there is a difference, for most, in what one does for oneself and when another does it - even a stranger,
To some there might be a kind of choice; a stranger, male of female, offers some i mediate threat but posdibly less ongoing threat. If the "therapy" is carried out in a specific location, "equipped" for the purpose in a "professional" manner that is less threatening than, ssy, a hotel toom or your own dwelling.
I have known women who doubt their capability to feel satisfaction because, due to a very demanding, selfish partner say, they have not done do for some time. A couple turned to other women as being less threatening and more understanding and sharing. I can understand that.
For pity's sake, it's just a hand job! I can understand it can be more fun with someone else, but why on earth pay for it?

For some payment puts in a barrier against them feeling responsibility for the other, especially for "satisfying" that other, or becoming involved in a more emotionally entangled relationship. This can protect their feelings of vulnerability and insecurity.
Of course, if you have never suffered those problems it is not so easy to understand.
