my tanks hold 300 gallons and the price here is US$4.00 per US gallon.  so its $1200 to fill empty tanks.

i dont have a functioni g fuel gauge so i just top it off every day, maybe 75 gallons. but i skipped a day because i was off, so im low.

ah well. at least im at a truck stop so thereis coffee

Annual checkup with my family doctor yesterday. I am doing well except for some low level OCPD.  The doctor got his education in Columbia.  His name is Velasquez.  He is one of the old school docs that spend enough time with his patients that he is much appreciated.  Alas I learned, after my session with him, that he is leaving the large clinic that I use for medical advice or treatment.

The clinic is huge and well regarded. They have as many as 200 doctors with every specialties one could need. They also have general practitioners who point us in the appropriate directions toward specialists. Fabio Velasquez is one of those. He does referrals when he truly believes that we may need specialist care. 

The deal is that he spends too much time with his patients. In the big world of American medicine the physician is a business person who needs to spend no less time with his patients than critically  necessary.  That translates to seeing at least 30 patients per day. Many of them push the count to 40 per day. That is all a matter of billing and its' economics.  My insurance is billed for $193 per visit.  That is negotiated by the insurance carriers such that the actual pay to the clinic is in the neighborhood of $80.  That translates to a measly $2400 to $3200 per day.

I do not know why Dr. Velasquez is leaving the clinic but I suspect that it might be a corporate decision to let him go.  He is simply not generating enough revenue because he spends time with the patient in such a way that he does not see 30 or 40 patients per day. I am really sad that he is leaving.  He is a damned good guy who has run afoul of the way American Medical business is done.  The clinic with its 200 docs is then generating something like a half million a day in billing.

OK I get it.  They are there when I need them.  But the cost is working against the ordinary person who does not have adequate insurance to keep him or her out of debtors prison.

At the direction of my cardiologist specialist. I had a stress test procedure done a few months ago. That was a big deal that sent me through a whole raft of tests that established that my condition is more than satisfactory. The billing was $5600.  Sheesh! We gotta pay the medical practitioners well but do we have to bankrupt the ordinary citizen to do so? In fairness, I have to agree that I got off well with an out of pocket expense of about $480.  Thanks to my Medicare and Humana insurance benefit.

That is why I am being a grumpy old man.

my medical issues are less grumpy than yours

they cut open my eyeball yesterday and sucked out the lens. i wanted to be awake to watch but they gave me an anaesthetic that kept me conscious but not rational. so all i can remember is looking into bright light while red shapes came and went in my vision\. i asked if i would be able to play the violin after the procedure and they told me not unless i could play it before. they were on to me there.

but when i got home things were stil cloudy in that eye until i dropped in some steroid eyedrops about four hours later. that cut down the inflammation and my vision almost instantly cleared and became as good as that in the untreated left eye. like hitting a switch almost.

very impressive. before it was cloudy, with double vision in good light and haloes around light sources in bad light. lots of glare from the internal reflections. but thats all gone

so the vision is good but th problem is that the incision is still healing up and i am to refrain from doing anything that might stir dust into it and cause an infection, and i have realized that everything i do in my life is dirty.

i cant work in the shop, because it is dusty there. i cannot sweep the house. i cannot ride a motorcycle. i cannot clean out the basement. i cannot work under a car. i stay out of the henhouse and the goat barn. its raining so theres no dust outside but if there were id have to stay away from it. i never realized that my life consisted primarily of being dirty.

so im stuck in the house just sitting next to the propane heater and reading for a week. and bothering people on the internet.

icarus, is this velasquez maintaining a practice elsewhere? can you follow him, or is that not possible?

my whole life is dirty. now that i have a chance to clean it up i cant. so im reading everything there is on the internet about the AMAL T10GP2 carburetter

i was there three hours, of which about 20 minutes were actual procedure and the other was paperwork.

the opthamologist did eight that day. i asked what was typical and they said fourteen.

i dont know whether its a once per week thing for him or more infrequent, but he was running em through for sure. put a drop of ink on my forehead above th eeye in pre op so he wouldnt screw up and do the wrong one

fascinating. its been 24 hours and the worked on eye now has better acuity than the other
